Why are the survey forms numbered?

The survey forms are individually numbered for two reasons

  1. To ensure that survey forms are not copied and people only have one vote.
  2. The numbering of the survey forms let's us accurately record how many were posted out and how many were returned.

(No records are kept of the numbered survey forms.  If you fill in the survey form and post it back to Council, your identity remains completely anonymous.)

Do I fill in the survey form or the submission form or both?

You can fill in one or both.  The survey form will be counted with all the other returned survey forms, and is a great way to express your opinion.

If you feel strongly on the subject, it is important to fill in the submission form as well.  You can also present your submission at a Council hearing if you wish.

I didn't get a survey form?

Survey forms will be sent only to owners of properties connected to the Waipukurau water network.

If you do not own your property in Waipukurau, but will be affected by the decision e.g. student, tenant, business - you can still have your say by completing a submission form.

If you do own property in Waipukurau and did not receive a survey form, please contact the CHB District Council on 06 85 78 060.

Lost the survey form?

If you have lost your survey form, but still want to have your say, please contact the CHB District Council on 06 85 78 060.

Lost the post-paid envelope?

Drop in your completed survey form into your nearest Library or Council office in Waipawa during business hours.

Where do I get a submission form?

If you didn't get a survey and submission form in the post, you can submit online at www.chbdc.govt.nz, or pick up a submission form from your nearest Library or the CHB District Council office in Waipawa.  If you've lost a survey form, please contact Council.

Can I speak about my submission in front of the Councillors?

You are welcome to have your say about the issue of fluoridating the Waipukurau water supply at a Council Hearing.  The Hearing date is to be advised. 

Why is the survey non-binding?

There are many things the Council must consider before making a decision to continue adding or stop adding fluoride to the Waipukurau water supply.

Many people may be affected by the decision because they work or go to school in Waipukurau and they drink the water, but they may live outside Waipukurau. 

There is no practical way to capture their vote on a survey - therefore the survey results can only be used as an indicator of public opinion.

The survey results as well as open submissions and advice from various experts will enable Councillors to make the most informed decision.
